Patients identify trusted people they can contact when they need support, including family members, friends, providers, crisis lines, or community resources.

For Healthcare Partners

Alliance Behavioral Health Integrated works with healthcare referral partners to support patients who may benefit from non-emergency crisis prevention planning, behavioral health follow-up, safety planning, care coordination, therapy, medication management, addiction treatment support, or stability support after an emergency room visit, hospitalization, relapse concern, or change in symptoms.

Referrals may be appropriate when a patient needs help recognizing early warning signs, understanding emotional or situational triggers, building coping strategies, coordinating follow-up care, or staying connected to treatment before symptoms escalate. Our team can help support continuity of care while protecting patient privacy and following applicable HIPAA requirements.

Healthcare referral partners may include primary care providers, hospitals, emergency departments, psychiatrists, therapists, counselors, substance use treatment providers, rehab programs, IOP programs, case managers, care coordinators, discharge planners, social workers, pain management providers, and insurance or care management teams.
For Community Partners & Organizations
Alliance BHI also works with community partners and organizations that may identify individuals who need non-emergency behavioral health support, crisis prevention planning, safety planning, addiction recovery support, or help connecting with appropriate care.
Â
Patients may come to support through many community pathways, including churches, faith-based programs, courts, DMV-related requirements, DUI programs, recovery organizations, shelters, schools, family support systems, and local agencies.

Community referrals may be appropriate when someone is struggling with emotional distress, worsening symptoms, family or legal stress, substance use relapse concerns, difficulty following through with treatment, or the need for coordinated behavioral health support. Alliance BHI can help review the request and connect the individual with the appropriate next step when clinically appropriate.

Community partners may include churches and faith-based organizations, community nonprofits, courts, probation programs, DMV-related programs, DUI schools, legal advocates, shelters, housing programs, recovery organizations, family support organizations, schools, employers, social service agencies, veteran support programs, and other community-based organizations.
